CLASS PACKAGES TERMS & CONDITIONS
Membership, class packages and session fees are non-refundable and non-transferable. Membership is valid for 1 year while sessions for class packages are valid for 30 days upon purchase. Membership and class packages cannot be put on hold (or freeze once activated). Make up classes can be scheduled anytime during the validity of the package. If for any reason you are unable to attend a scheduled session, notice must be given at least 12 hours before the class. Otherwise the session will be considered utilized. Payment must be made in full prior to the commencement of the first session.
PARENT CONSENT, ASSUMPTION OF RISK, AND LIABILITY WAIVER
1. Participation Consent
I, the undersigned parent or legal guardian, hereby grant permission for my child to participate in training programs, classes, conditioning sessions, and related activities conducted by HMD Gymnastics.
I understand that gymnastics is a physically demanding sport requiring strength, flexibility, coordination, and physical instruction from trained coaches.
2. Assumption of Risk
I acknowledge that participation in gymnastics involves inherent risks, which may include but are not limited to:
• muscle strains or fatigue
• sprains, fractures, or joint injuries
• falls from gymnastics equipment
• accidental contact with equipment or other participants
• other injuries that may occur during training or physical activity
I voluntarily allow my child to participate and accept and assume all inherent risks associated with gymnastics activities.
3. Physical Instruction, Spotting, and Incidental Contact
I understand that gymnastics training often requires physical spotting, support, and correction from coaches in order to:
• prevent falls
• assist with learning techniques
• maintain proper body alignment and balance
• ensure athlete safety
During instruction, a coach may physically guide or support the athlete’s body, which may involve contact with different parts of the body necessary for instruction or safety.
I acknowledge that incidental contact with sensitive or private body areas may occasionally occur during spotting or correction, but such contact is brief, necessary for instruction or safety, and not malicious or inappropriate in nature.
I hereby consent to such physical assistance by qualified HMD Gymnastics coaches during training sessions.
4. Health Declaration
I confirm that, to the best of my knowledge, my child:
• is physically fit to participate in gymnastics activities
• does not have medical conditions that would make participation unsafe
If my child has any medical condition, injury, allergy, or special health concern, I agree to disclose this information to HMD Gymnastics prior to participation.
5. Medical Authorization
In the event of an accident, injury, or medical emergency involving my child, I authorize HMD Gymnastics and its staff to obtain necessary medical treatment, including first aid and emergency medical services.
I understand that reasonable efforts will be made to contact me or the emergency contact person before any medical action is taken whenever possible.
6. Compliance with Gym Rules
I agree that my child will comply with all gym rules, safety policies, and coach instructions while participating in activities at HMD Gymnastics.
I understand that failure to follow safety rules may result in temporary suspension or removal from activities for the safety of all participants.
7. Release of Liability
To the fullest extent permitted by applicable law, I release and hold harmless HMD Gymnastics, its owners, officers, coaches, employees, agents, and representatives from liability for injuries, damages, or losses arising from my child’s participation in gymnastics activities except in cases of gross negligence or willful misconduct.
8. Indemnity
I agree to indemnify and hold harmless HMD Gymnastics from any claims, demands, or legal actions brought by or on behalf of my child arising from participation in gymnastics activities, except where caused by gross negligence or intentional misconduct.
9. Photography and Media Consent
I authorize HMD Gymnastics to take photographs or videos of my child for the following purposes:

10. Parent/Guardian Acknowledgment
By clicking the I AGREE button, I confirm that:
• I have read and understood the Class Package Terms & Conditions and Parent Consent and Liability Waiver.
